One question that'd help us scope it: would you want the NPS score to just live on the form, or would you also want to filter and segment your testimonials by NPS score afterwards?

On top of this, when a positive rating is selected on this form, it could prompt the end-user for a testimonial. Of course, if the end-user offers a negative NPS rating, a different form might appear, prompting them for detailed feedback and capturing the problem.
