Reorder questions to have text first and then video

How do I reorder the questions in the 1.0 form? It asks for a video testimonial first, I’d like to swap and reorder the questions, and ask for a text testimonial first (so can offer $20 voucher) and then move forward to also add a video testimonial next (to give them a bonus $50gift voucher) so they can earn up to $70 for a text and a video review. At the moment it only says video or text? It should allow you to be able to leave both.

Your forms should have AND or IF conditions. It only allows you to only do video or text. It should allow you to do both!

This will help to gather more reviews and remove customer friction.
