Star rating per question needed

Hello, I tried to set up a customer feedback survey and although this is a great design, it did not offer me what I needed. I need a survey that I could build to ask clients star ratings per question. It only allows video or text response per question after the initial 1st star rated question. So is this could be built in- that would be awesome!
