2.0 Forms Feedback

Hey, I’ve been playing around with the new 2.0 Form layout and it’s looking great.
One thing that is missing is for Negative Feedback (1 - 3) Rating, there is no option to include the About You Page. It seems to only show if people were to leave a positive testimonial.

From a customer service perspective it is really important to be able to understand who has left negative feedback so we can see how we can help them.
